All mg/kg dosing is based on the amoxicillin component. See important notes below around the appropriate specification of product to prescribe/dispense.
1 to 3 months of age: 15 mg/kg/dose PO BID Use 25 mg/mL suspension (approx. 4:1 amoxicillin: clavulanate)
Greater than 3 months: 13 -16 mg/kg/dose PO TID Maximum: 500 mg/dose
Less than 35 kg: Use the 80 mg/mL suspension (approx. 7:1 amoxicillin:clavulante)
Greater than or equal to 35 kg: Use 500 mg tablets OR 80 mg/mL suspension (approx. 7:1 amoxicillin:clavulante)
Outpatient
Treatment failure with amoxicillin or otitis-conjunctivitis syndrome
Acute Bacterial: fever greater than 39 degrees or treatment failure with amoxicillin
Periorbital Cellulitis
Aspiration Pneumonia
Using a product with incorrect amoxicillin-clavulanate could result in sub-therapeutic clavulanate concentrations or severe diarrhea. If possible, limit clavulanate to less than 6.4 -10 mg/kg/24h to reduce GI side effects. Based upon usual dosing in pediatric patients less than 35 kg, use of the 7:1 ratio suspension provides 6.4-8.6 mg/kg/24h of clavulanate and 4:1 ratio suspension provides 11.2-15 mg/kg/24h of clavulanate.
