May consider deferring antimicrobial therapy if single positive blood culture and absence of clinical evidence of sepsis.
If IV catheter is likely source, removal of catheter alone may be sufficient
Sources include gastrointestinal tract, urinary tract, intravascular catheters and wounds.
Can be associated with endocarditis.
Risk of endocarditis fairly low unless valvular disease and rare causes septic shock
Vancomycin 15-20mg/kg IV Q8-12H
Dose to a target trough level of 15-20mg/L
SWITCH TO
Ampicillin 2g IV Q4H
*If susceptible.
Uncomplicated: 5-7 days.
Tailor duration based on source of bacteremia.
