Gentamicin, if selected, should only be added for the first two weeks of treatment.
Penicillin G 24 million units IV per 24 hr either continuously or in 4-6 equally divided doses
OR
Ceftriaxone 2g IV/IM q24h
WITH OR WITHOUT
Gentamicin 3 mg/kg IV/IM q24h
Penicillin or ceftriaxone in combination with gentamicin have not shown to have superior cure rates compared with monotherapy for patients with highly susceptible strain
Gentamicin therapy should not be administered to patients with CrCl under30 mL/min.
Gentamicin is added for the first two weeks of treatment.
ALTERNATIVELY, CONSIDER
Vancomycin 15-20 mg/kg IV q8-12h
Dose should be adjusted to obtain
serum trough levels of 15-20 mg/L. Peak levels are not routinely indicated.
