Penicillin G 24 million units IV per 24 hr either continuously or in 4-6 equally divided doses
PLUS
Gentamicin 3 mg/kg IV/IM q24h
Gentamicin is added to the penicillin regimen for the first two weeks.
ALTERNATIVELY, CONSIDER
Ceftriaxone 2g IV/IM q24h
Ceftriaxone monotherapy is reasonable to consider if isolate is susceptible
OR
Vancomycin 15-20 mg/kg IV q8-12h
Dose should be adjusted to obtain serum trough levels of 15-20 mg/L. Peak levels are not routinely indicated.
