Antimicrobials
Preferred in most patients >65 yr and in those with impairment of 8th cranial nerve or renal function
Duration: 4 weeks
EITHER
Penicillin G 12-18 million units IV per 24 hr either continuously or in 4 or 6 equally divided doses
OR
Ceftriaxone 2 g
IV/IM q24h
2 week regimen is not intended for patients with:
Known cardiac or extracardiac abscess
CrCl <20 mL/min
Impaired 8th cranial nerve
Abiotrophia, Granulicatella,or Gemella spp infection
Duration: 2 weeks
Penicillin OR Ceftriaxone as above
PLUS
Gentamicin 3 mg/kg IV/IM q24h
Should not be administered to patients with CrCl under 30 mL/min.
Reserved for patients unable to tolerate penicillin or ceftriaxone
Vancomycin 15-20 mg/kg IV q8-12h
Dose should be adjusted to obtain
serum trough levels of 15-20 mg/L. Peak levels are not routinely indicated.
