Streptococcal - Native - Highly Susceptible

4 Week Regimen

Preferred in most patients >65 yr and in those with impairment of 8th cranial nerve or renal function

Duration: 4 weeks

EITHER

OR

2 Week Regimen

2 week regimen is not intended for patients with:

  • Known cardiac or extracardiac abscess

  • CrCl <20 mL/min

  • Impaired 8th cranial nerve

  • Abiotrophia, Granulicatella,or Gemella spp infection

Duration: 2 weeks

Penicillin OR Ceftriaxone as above

PLUS

Should not be administered to patients with CrCl under 30 mL/min.

Vancomycin Regimen

Reserved for patients unable to tolerate penicillin or ceftriaxone

Duration: 4 weeks

Dose should be adjusted to obtain

serum trough levels of 15-20 mg/L. Peak levels are not routinely indicated.
