Greater than 2kg
50mg/kg/dose IV/IM q8h-q12h

Smaller doses and longer intervals of administration may be advisable for very low–birth weight neonates (less than 2000g).

Neonates 8-28 Days of Age

Less than 1.2kg
50mg/kg/dose IV/IM q12h

Greater than 1.2kg
