Vancomycin - Meningitis Specific Dosing

Note

Maintain serum trough concentrations of 15–20 mg/mL.

Neonates

PMA less than 27 wks
24mg/kg/dose IV q24h

PMA 27-36wks
18mg/kg/dose IV q8h

PMA greater than or equal to 37wks
15mg/kg/dose IV q6h

Smaller doses and longer intervals of administration may be advisable for very low–birth weight neonates (less than 2000g).

Infants and Children

15mg/kg/dose IV q6h

Max: 1g/dose prior to TDM

Adults

Loading dose of 25-30 mg/kg then,
15-20 mg/kg/dose IV q8-12h
