Cefotaxime - Meningitis Specific Dosing

Neonates 0-7 Days of Age

Less than 2kg
50mg/kg/dose IV/IM q12h

Greater than 2kg
50mg/kg/dose IV/IM q8h-q12h

Smaller doses and longer intervals of administration may be advisable for very low–birth weight neonates (less than 2000g).

Neonates 8-28 Days of Age

Less than 1.2kg
50mg/kg/dose IV/IM q12h

Between 1.2-2kg
50mg/kg/dose IV/IM q8h

Greater than 2kg
50mg/kg/dose IV/IM q6h-q8h

Infants and Children

50mg/kg/dose IV/IM q6h

Up to 75mg/kg/dose may be used in selected cases in consultation with ID.

Dose limit: 12 g/day
