Gentamicin - Meningitis Specific Dosing

Note

Need to monitor peak and trough serum concentrations.

Neonates 0-7 Days of Age

Less than 34 week PMA: 3 mg/kg IV q24h

Greater than or equal to 34 weeks: 3 mg/kg IV q18h

Neonates 8-28 Days of Age

Greater than or equal to 1 kg and less than 37 weeks: 2.5 mg/kg IV q12h

Greater than or equal to 1 kg and greater than and equal to 37 weeks: 2.5 mg/kg IV q8H

Infants and Children

Synergy: 1 mg/kg IV/IM q8h

Synergy with beta-lactams for gram positive infections

Adults

Synergy: 1.7 mg/kg IV q8h
