Low Risk Regimens

Empiric Antimicrobial Regimen - PO

Ciprofloxacin 750 mg PO q12h

AND

Amoxicillin/Clavulanate
500/125 mg PO q8h
or
875/125 mg PO q12h

If patient on an oral fluoroquinolone for prophylaxis, initiate IV therapy below (High Risk Regimen)

Empiric Antimicrobial Regimen - IV

Pipercillin-Tazobactam 4.5 g IV q6h*

*Preferred regimen

OR

Meropenem 500 mg IV q6h**

**First choice if known history of ESBL infection
