β-lactam Allergy

Vancomycin 20 mg/kg IV

Redosing Dose & Interval

15 mg/kg q12h

PLUS

Gentamicin 5 mg/kg IV

Redosing Dose & Interval

2 mg/kg q6h

Post-op Antibiotics

None unless one of the following:

  • Acute cholecystitis

  • Gangrene

  • Perforated gallbladder
