Duration of Therapy

Greater than or equal to 21 Days

Regimen of Choice

PLUS EITHER

  • For 3 - 7 days

  • Need to monitor peak and trough serum concentrations

  • Synergy with beta-lactams for gram positive infections

OR

  • For 3 - 7 days

  • Need to monitor peak and trough serum concentrations

  • Synergy with beta-lactams for gram positive infections

PCN Allergy

Dosage based on trimethoprim component

Penicillin/Beta-lactam Allergy

If the patient has a true history of IgE mediated penicillin or beta-lactam allergy, the risks versus benefit of empiric treatment of suspected meningitis with appropriate antimicrobials should be discussed.
