Ceftriaxone 50-75 mg/kg/day IV in 1 to 2 divided doses. Max 2g/day
Cefotaxime should be used instead of ceftriaxone if hyperbilirubinemia is present or if patient is receiving calcium-containing IV solutions.

Metronidazole 10 mg/kg IV/PO q8h Max 4g/day IV; 2g/day PO

Piperacillin-Tazobactam
1 Month to Less Than 6 months 80 mg piperacillin/kg IV q6h
Over 6 Months 100 mg piperacillin/kg IV q6h
