Strep MIC<0.12 Treatment / Infective Endocarditis

Native Valve

Ampicillin 2 g IV every 4 hr x 4 wks

OR

Ceftriaxone 2 g IV every 24 hr*

*Alternative in patients with nonsevere PCN allergy

Prosthetic Valve

Ampicillin 2 gm IV every 4 hr x 6 wks

OR

Ceftriaxone 2 g IV every 24 hr* x 6 wks

*Alternative in patients with nonsevere PCN allergy

With OR Without

Gentamicin 1 mg/kg IV every 8 hr x 2 wks
