Enterococcal / UTI

E. Faecalis

Almost all E. faecalis isolates are susceptible to Amoxicillin OR Ampicillin and should be treated with these agents.

E. Faecalis Treatment

Amoxicillin 500 mg PO TID

OR

Ampicillin 1 g IV every 6 hr

PCN Allergy

Nitrofurantoin 100 mg PO BID

E. Faecium

Commonly Vancomycin resistant

If susceptible, Amoxicillin or Ampicillin is preferred.

E. Faecium Treatment

Amoxicillin 500 mg PO TID

OR

Ampicillin 1 g IV every 6 hr

OR

Vancomycin

OR

Nitrofurantoin 100 mg PO BID

OR

Linezolid 600 mg PO/IV every 12 hr
