If Pseudomonas is not recovered, therapy should be de-escalated to agents with a narrower spectrum of activity.
Catheters as less commonly the source of the infection. However, most advocate for their removal if the catheter is the source.
Gram-negative endocarditis is not common and routine echocardiography is discouraged.
Cefepime 2 g IV every 8 hr
OR
Zosyn 3.375 g IV every 8 hr (4 hr infusion)
Aztreonam 2 g IV every 8 hr
10-14 days
