Can be a contaminate. Draw repeat cultures prior to starting therapy to confirm infection.

In 2017, 74% of E. Faecium isolates at Memorial were resistant to vancomycin.

If the isolate happens to be susceptible to ampicillin or vancomycin, these agents used be used.

Linezolid or Daptomycin require completion of the Restricted Antimicrobial Order form, unless being ordered by an Infectious Diseases physician.

Do not use gentamicin for synergy if the lab lab reports no synergy with gentamicin; doing so will only increase the risk of nephrotoxicity without clinical benefit.
