Uncomplicated UTI

Levofloxacin is not listed as an empiric treatment recommendation at BMH for patients with non-catheter associated UTI due to low rate of E.coli susceptibility.

Per BMH policy Levofloxacin will automatically be intherchanged if ordered emprically for UTI, except in cases of SEVERE allergy.

Common Pathogens

E. coli

S. saprophyticus

Empiric Therapy

Cephalexin 500 mg PO BID

OR

Ceftriaxone 1 g IV every 24 hr

Cephalosporin / Severe PCN Allergy

Nitrofurantoin 100 mg PO BID
