Pyelonephritis

Signs and symptoms (e.g. fever, flank pain) AND pyuria AND positive urine culture ≥ 100,000 colonies

Many patients will have other evidence of upper tract disease (i.e. leukocytosis, WBC casts, or abnormal imaging).
