Elevation of the affected area is recommended
Draw a line around the margins of the infection with a marker to better monitor day-to-day changes
Vancomycin
PLUS
Ampicillin/Sulbactam 3 g IV every 6 hr
Ceftriaxone 1 g IV every 24 hr
Metronidazole 500 mg IV every 8 hr
Referral to podiatry as outpatient.
Cellulitis often appears to get worse within the first 24 hours of antibiotics despite adequate therapy. If pain is stable or improving, continue recommended antibiotics.
At 72 hours, if 20% resolution of erythema, switch to PO and evaluate for discharge.
