Immunocompromised?
This patient is at risk for unusual pathogens and/or rapid deterioration depending on degree of immunosuppression. Depending on the severity of illness, consider Infectious Disease consult.
Droplet / Contact Precautions
Significant travel Hx? This patient is at risk for unusual pathogens. Depending on the severity of illness and location of travel, consider contact and droplet precautions and Infectious Disease consult.
Influenza season and influenza-like illness
The season typically runs from Nov. to April, but there may be significant variation.
Clinical features: fever, headache, myalgia, malaise, non-productive cough, sore throat and rhinorrhea
Bronchiectasis or COPD with frequent Abx use
Severe pneumonia (e.g. sepsis AND FiO2>50% OR large/rapidly progressive infiltrates)
Sepsis
Both: SIRS criteria (Two of HR>90, tachypnea, Temp >38 or <36, WBC >12 or <4) AND Suspected/confirmed infection,
Recent antibiotic use < 3 months, end-stage renal disease, or IV drug use, colonized with MRSA
