Immunocompromised?
This patient is at risk for unusual pathogens and/or rapid deterioration depending on degree of immunosuppression. Depending on the severity of illness, consider Infectious Disease consult.
Droplet / Contact Precautions
Significant travel Hx? This patient is at risk for unusual pathogens. Depending on the severity of illness and location of travel, consider contact and droplet precautions and Infectious Disease consult.
Shorter duration antimicrobial therapy may be appropriate in some cases

Influenza season and influenza-like illness
The season typically runs from Nov. to April, but there may be significant variation.
Clinical features: fever, headache, myalgia, malaise, non-productive cough, sore throat and rhinorrhea
Bronchiectasis or COPD
Antibiotic use within past 3 months or major comorbidity (e.g. heart, lung, liver, renal, DM, malignancy, asplenia)
Presentation concerning for tuberculosis
Tuberculosis clinical features
Subacute illness, weight loss, night sweats, hemoptysis, travel to or emigration from endemic area, upper lobe infiltrates, cavitating lesions.
