Anaerobic species (most common Bacteroides)

Aerobic gram-negative enteric organisms species (most common E. coli)

Regimen of Choice

Cefazolin 2g IV

Redosing interval

q4h

PLUS

Metronidazole 500mg IV

Redosing interval

q8h

β-lactam Allergy

Metronidazole 500 mg IV

Redosing interval

q8h

Gentamicin 5 mg/kg IV

Redosing Dose & Interval

Not necessary unless significant blood loss as this is q24h dosage regimen

Post-op Antibiotics

None unless one of the following:

  • Perforation

  • Gangrene

  • Peritonitis

  • Abscess
