Timing of Antibiotics

Antibiotics are to be given 1-2 hrs prior to procedure

Regimen of Choice

Trimethoprim-Sulfamethoxazole DS 1 tablet PO

OR

Ciprofloxacin 500 mg PO*

*Reserve for patient with sulfa-allergy

OR

Cefazolin 2 g IV

Redosing interval:

q4h

β-lactam Allergy

Trimethoprim-Sulfamethoxazole DS 1 tablet PO

OR

Ciprofloxacin 500 mg PO*

*Reserve for patient with sulfa-allergy

OR

Ciprofloxacin 400 mg IV

Redosing interval

q6h

OR

Gentamicin 5 mg/kg IV

Redosing Dose & Interval

Not necessary unless significant blood loss as this is q24h dosage regimen

with or without

Clindamycin 900 mg IV

Redosing interval

q6h

Post-op Antibiotics

24 hours
