  • Ensure adequate blood culture data collected.

Suggested Regimen

Vancomycin IV loading dose followed by maintenance for target trough of 15-20 ug/mL

If patient cannot be dosed using nomogram
contact Pharmacy.

Ceftriaxone 2g IV daily
