Minimum 5 days therapy
In patients with negative finalized culture(s) AND no conclusive evidence of infection, proceed to de-escalation only after a minimum of 5 days of therapy.
However, If cultures are negative, patient afebrile ≥48 hours, AND their reason for hospital admission was not febrile neutropenia, consider de-escalation to fluoroquinolone prior to completion of 5 days of empiric therapy if patient was not previously on fluoroquinolone prophylaxis
