Prosthetic Valve IE: Empiric Abx

Prior to starting antimicrobials

Ensure adequate blood culture data has been collected.

Suggested Empiric Regimen

Ceftriaxone 2g IV q24h

AND

Vancomycin 25mg/kg IV load followed by 15mg/kg IV q12h maintenance for a target trough of 15-20mg/L

Strongly consider Infectious Diseases consultation in patients who are hemodynamically unstable for consideration of gentamicin and/or rifampin empirically for suspected prosthetic valve IE.
