CeFAZolin IV 30 mg/kg/dose (Max 2000 mg/dose)*
*The maximum adult IV dose for CeFAZolin in patients ≥ 120 kg is 3 g/dose.

Maximum Post-Operative Duration
If MRSA positive, vancomycin should be given in addition to cefazolin.
Vancomycin IV 15 mg/kg/dose (Max 1000 mg/dose)

If sinuses opened during surgery, ADD:
Metronidazole IV 10 mg/kg/dose (Max 500 mg/dose)
