Antimicrobials
If no incision through the oral/nasal/pharyngeal mucosa, pre-operative antimicrobials not required.
CeFAZolin IV 30 mg/kg/dose (Max 2000 mg/dose)*
*The maximum adult IV dose for
CeFAZolin in patients ≥ 120 kg is 3 g/dose.
Repeat Dosing During Procedure
q4h
If MRSA positive, vancomycin should be given in addition to cefazolin.
Vancomycin IV 15 mg/kg/dose (Max 1000 mg/dose)
q6h
Not required.
For some orthognathic surgeries, post operative antibiotic prophylaxis may be given for ≤ 24 hours
