Note

Patients with severe infection are at risk of abdominal compartment syndrome - monitor intrabdominal pressures if possible

Abdominal compartment syndrome (ACS) is defined as intra-abdominal pressure >20mmHg and new organ failure (renal, pulmonary or hemodynamic)

If ACS is suspected there are many treatment options available although a surgical consult should be sought to guide supportive measures and to assess for the possibility of a laparotomy

Empiric Antimicrobial Regimen

Duration of therapy depends on clinical course

In patients who fail to improve, surgical debridement of necrosis may be required and consultation with surgical team recommended

  • Colonization of multiple body sites

  • Immunocompromise

  • Broad-spectrum antibiotic use

  • Abdominal surgery

  • Central venous catheters

  • TPN
