Oral Stepdown and Discharge

Criteria

  • Clinically improving *

At least 2-4 days of IV treatment
* Two consecutive decreases in CRP
* The patient can tolerate oral therapy
* The patient tolerates a dose of the home oral therapy in the hospital
* Highly encourage the concierge service supplying a 30 day supply of the antibiotic to ensure taste preference (same as inpatient dose) and reduce compliance issues related to travel to the pharmacy
* Weekly follow up with ID and follow up with pediatric orthopedics
