Piperacillin-tazobactam 75 mg/kg (piperacillin component) IV Q6H
PLUS
Vancomycin 20 mg/kg IV Q6H
Clindamycin 7 to 13 mg/kg IV Q8H
Ciprofloxacin 7.5 to 15 mg/kg IV Q12H
*Avoid all beta-lactams antibiotics (ie cephalosporins, carbapenems) if anaphylaxis to penicillin
