Empiric Antibiotic Options

Cephalexin 10 to 25 mg/kg PO Q6H

Penicillin Allergy*

Cotrimoxazole 4 mg TMP/kg PO BID

*Avoid all beta-lactams antibiotics (ie cephalosporins, carbapenems) if anaphylaxis to penicillin
