Ampicillin 50 to 100 mg/kg IV Q6H

Clarithromycin 7.5 mg/kg PO BID

Penicillin Allergy*

Cefuroxime 25 to 50 mg/kg IV Q8H

Clarithromycin 7.5 mg/kg PO BID

*Avoid all beta-lactams antibiotics (ie cephalosporins, carbapenems) if anaphylaxis to penicillin
