Most likely pathogens

Duration of therapy

7-10 days (longer if complications)

Suggested regimen

Ceftriaxone 50-75 mg/kg/dose IV q24h
Maximum: 2 grams/dose

AND if Mycoplasma suspected

Clarithromycin 7.5 mg/kg/dose PO BID
Maximum: 1 gram/24h

AND if Influenza suspected or proven,

Oseltamivir

Penicillin allergy

Ceftriaxone 50-75 mg/kg/dose IV q24h
Maximum: 2 grams/dose

AND if Mycoplasma suspected

Clarithromycin 7.5 mg/kg/dose PO BID
Maximum: 1 gram/24h
