7-14 days (depending on pathogen and clinical response)

Suggested Regimen

Piperacillin-Tazobactam
Preferred Dosing:
3 grams* IV q6h
*Injection 3/ 0.375 g vial containing: 3 g piperacillin and 0.375 g tazobactam

OR

Alternate Dosing:
4 grams* IV q8h
*Injection 4/ 0.5 g vial containing: 4 g piperacillin

and 0.5 g tazobactam

Penicillin Allergy

Ceftriaxone 1 g IV q24h

AND

Metronidazole 500 mg IV q12h
