Clindamycin Peds Dosing

IV: 25-40 mg/kg/24h IV divided q6-8h
Maximum/dose: 900 mg/dose
Maximum/24h: 3600 mg/24h in severe infections
Dosing applies to patients with cystic fibrosis

PO: 20-30 mg/kg/24h PO divided TID -QID
Maximum: 1800 mg/24h
Use higher doses in severe infections.
