Post natal age less than or equal to 28 days: 50 mg/kg/dose IV q12h
Post natal age greater than 29 days: 50 mg/kg/dose IV q8h
Post natal age less than or equal to 7 days: 50 mg/kg/dose IV q12h
Post natal age greater than or equal to 8 days: 50 mg/kg/dose IV q8h
Post natal age less than or equal to 7 days: 50 mg/kg/dose IV q8-12h
Post natal age greater than or equal to 8 days: 50 mg/kg/dose IV q6-8h
Less than 4 weeks of age
Less than or equal to 30 days
Less than 72 hours admitted to NICU
