Current data are insufficient to recommend a specific dose of oseltamivir for premature infants
An infectious disease physician or clinical pharmacist should be consulted.
Current weight-based dosing recommendations for oral oseltamivir are not appropriate for premature infants, who may have slower clearance because of immature renal function.
Dosages recommended for full-term infants can lead to very high drug concentrations.
The Centers for Disease Control and Prevention and the American Academy of Pediatrics (Committee on Infectious Diseases, 2016) recommend dosing based on limited data from the National Institute of Allergy and Infectious Diseases Collaborative Antiviral Study Group:
1.5 mg/kg/dose PO BID
