Streptococcus pneumoniae

Empiric Treatment (IV)

Ceftriaxone 1g IV q24

OR

Azithromycin 500mg IV daily

Empiric Treatment (PO)

Amoxicillin 1g PO TID

OR

Cefpodoxime 200 mg PO BID

OR

Azithromycin 500 mg PO daily

Note

Levofloxacin should not be used routinely as step-down therapy for pneumococcal pneumonia if the organism is susceptible to beta-lactams.

Duration

Treatment for 5-10 days based on clinical stability.
