CAP in Hospitalized Patients

Empiric Treatment - Non ICU

Ceftriaxone 1g IV q24h

AND EITHER

Azithromycin 500mg PO/IV 1x daily

OR

Doxycycline 100mg PO/IV 2x daily

Levofloxacin 750mg PO/IV q24h

5-7 Days

In non-critically ill patients, switch to oral agents as soon as patient is clinically improving and eating.

IV therapy does not need to be continued until discharge.

It is not necessary to start all CAP patients on IV therapy if they can tolerate oral therapy, even if hospitalized.
