Treat for a minimum of 5 days (include therapy before oral switch).

Therapy can be stopped after the patient is afebrile for 48-72 hours and has no more than one of the following signs and symptoms: HR > 100 bpm, RR > 24 breaths/min, BP < 90 mmHg, O2 sat < 90%, altered mental status.

Therapy > 5 days without a clinical reason should be avoided.
