ID consultation is recommended
Surgical consultation as applicable for debridement
Penicillin G 4 million units IV Q4H
PLUS*
Clindamycin 900mg IV Q8H
*If signs of hemodynamic instability
Minimum 14 days from negative blood culture.
Tailor duration based on source of bacteremia.
Most common source is SSTI. Others include:
Pharyngitis
Pneumonia
Postpartum endometritis.
When the suspected source of infection may be polymicrobial then broader spectrum coverage is recommended.
May narrow to monotherapy (penicillin preferred) once there are no further signs of systemic infection and no further surgical intervention (e.g. if requires debridement or fasciotomy).
