In general, bacteremia does not occur spontaneously without infection at another site.

Therefore, determining the source of infection helps to guide the selection of empiric therapy to ensure coverage of most likely causative organism(s).

Empiric selection may require broader coverage than just for what is grown in the blood if the source of bacteremia is from an area that tends to be polymicrobial such as the gut.
