Cardiac valve replacement may be necessary as antimicrobial cure rate may be less than 50%.

Duration

More than 6 weeks

Antibiotic Regimen

Linezolid 600 mg PO q12hr (IV if oral not feasible).

OR

Daptomycin 10-12 mg/kg IV q24h
