Guideline

The clinical nurse will assess ALC/CCC patient’s immunization status upon admission and document in the electronic health record (EHR).

For patients that require immunization the Prescriber will provide patient/substitute decision maker (SDM) with information about the relevant immunizations and will obtain and document informed consent. The Prescriber will write an order for the required vaccines. If consent is not given by the patient or SDM, the Prescriber or clinical nurse will document in the EHR.

The clinical nurse will assess the patient’s/SDM’s knowledge level regarding the purpose and importance of vaccination, possible adverse side effects, procedure for vaccination, and provide additional health teaching as needed. The clinical nurse will document relevant details of vaccine administration in the eMAR, as outlined in this policy. A record of immunization will be provided to the patient.
