Notes

Empiric therapy should be initiated for a suspected UTI and positive urinalysis in those that present with:

  • Fever (especially greater than 39°C or temperature that has been elevated for more than 48 hours)

  • Ill appearance

  • Costovertebral angle tenderness

  • Known immune deficiency

  • Known urologic abnormality

Usual indications for hospitalization and/or parenteral therapy include:

  • Age <2 months

  • Sepsis (e.g. toxic appearance, hypotension, poor capillary refill)

  • Immunocompromised patient

  • Vomiting or inability to tolerate oral medication

  • Lack of adequate outpatient follow-up (eg, no telephone, live far from hospital)

  • Failure to respond to outpatient therapy

  • Genitourinary abnormalities
