Empiric Therapy

Acyclovir

PMA less than 32 weeks and weight less than 1200g:

20mg/kg/dose IV Q12H

PMA over 32 weeks and weight 1200g or more:

20mg/kg/dose IV Q8H

Dose must be adjusted for neonates with renal impairment
