Second or Subsequent Recurrence

Note

Consult ID and choose one of the following options

PO/NG Vancomycin Tapered and Pulsed Regimen

Max 125 mg/dose

Vancomycin 10 mg/kg po QID x 10-14 days

THEN

Vancomycin 10 mg/kg po BID 7 days

THEN

Vancomycin 10 mg/kg po DAILY

x 7 days

THEN

Vancomycin 10 mg/kg every 2-3 days x 2-8 weeks

Fecal Microbiota Transplantation

Appropriate antibiotic treatments should be tried for at least 2 recurrences (i.e. total of 3 CDI episodes) before offering fecal microbiota transplantation.

Fecal microbiota transplantation is recommended for patients (including pediatrics) with multiple recurrences of CDI who have failed appropriate or standard antibiotic treatments.
