Herpes simplex virus

Adults

Acyclovir 10mg/kg IV q8h for 14-21 days

Renal Dosing

  • CrCl 25-50 mL/min: q12h

  • CrCl 10 to less than 25 mL/min: q24h

  • CrCl less than 10 mL/min: 50% of dose q24h

Neonates, Infants and Children

less than or equal to 32 wks PMA and less than 1200 g

greater than 32 wks PMA and greater than or equal to 1200 g

1 month to less than or equal 12 years of age

greater than 12 years of age
